PACKAGE TWO
Individual Counseling & Support
In a 1 hour consulting session I help people explore their loved one’s current situation and strength, and how we can complement clinical support with the proven communication skills and resources of Positive Approach to Care. It is also a time to help identify and troubleshoot issues within the confines of the dementia journey.
Every 1 hour consultation includes a follow up with resources based on needs.
These can be calls, zoom or in person.
PACKAGE THREE
May alternatively be billed hourly.
Transitional Support
This support is directed towards the person living with dementia. This advocating relationship is designed to empower the individual transitioning to a new living arrangement. It is a responsive way to address support during the transition especially in a memory care community. The skilled services provided are executed alongside the person living with dementia with the utmost respect and dignity for them.
Introducing the person to their community
Helping set up room in a “dementia friendly manner”
Help facilitate relationship building between staff and new resident
Help transitions with acceptance of new environment
Serve as a liaison with family during the difficult transitional period
